In another corner of Salem, Sarah Horton is in a serious psychological crisis.

After her relationship with Xander continues to fall apart, she seeks out Dr. Marlena Evans for therapy.

But what Sarah brings to the clinic is more than just emotional wounds – it’s also a terrifying secret about her best friend Sophia.

According to spoilers, Sarah believes Sophia is hiding something about the accident that caused her temporary memory loss.

During a therapy session, Sarah breaks down in tears and tells Marlena:

“I see her in the mirror. But it’s not Sophia. It’s… someone else in her body.”

Marlena is stunned, beginning to recognize dangerous psychological signs: hallucinations, identity disorder, or worse – Sophia is actually up to something.
